CASE SHEET: 49 yr old woman with fever Chief complaints: Fever since 7 days associated with nausea , body pains and myalgia HISTORY OF PRESENT ILLNESS: Apparently the patient was  asymptomatic 7 days ago but then had fever fir which she took antibiotics prescribed by local RMP. But due to persistent fever zhe visted Mamatha hospital, Bhongiri and the tests confirmed thrombocytopenia . CASE SHEET: Chief complaints: SOB since a month , cough with sputum , weakness since a week, decreased urine output since a month  HISTORY OF PRESENT ILLNESS: Apparently the patient was asymptomatic a month ago but then started experiencing SOB and decreased urine output. CASE SHEET: Chief complaints: Stools with blood not associated with pain intermittent since one and half year.  HISTORY OF PRESENT ILLNESS: The patient noticed stools with blood intially one and half year ago. Blood with stool appeared intermittently ( once in 2-3 months) . The patient sometimes had constipation and something had watery stools. He also experienced SOB on exertion during physical activities like climbing stairs. The patient had fever since a week due to which he visited a local hospital and got some investigations including a colonoscopy .
